Our Story
Summit began in October 2005.
The people who started Summit had already spent 12 years in home health. They wanted a better way to join clinical care with customer service, and to treat patients, families, and referring medical professionals as customers.
Summit became Medicare and Medicaid certified in December 2005. The team includes physical therapists, registered nurses, and certified nursing assistants, and has grown to more than 50 people caring for seniors and other homebound patients in Southern Utah.

How We Care
Most of our patients need care on a regular basis and have difficulty leaving home. We visit them at home and teach them how to take care of themselves, including medication and IV therapy.
A Smaller Team
Because of our size, we can move faster than a large organization. We work with physicians’ offices and other referral sources to finish paperwork and see patients within a couple of days of the referral.
Why Choose Summit
- PT/INR protocol followed with the primary care physician
- PhD, RN clinical liaison on staff
- Nurses educated in home health and hospice care
- Certified cardiac and wound care nurses
- Patient and family communication is a primary focus
- Continuity of care
- 99% patient satisfaction
