About Home Health and Hospice

About Us

Summit Home Health & Hospice began in October 2005 with employees working in the home health industry for 12 years. In the beginning, Summit Home Health & Hospice observed that there should be a better way to integrate patient care with customer service. Summit Home Health & Hospice aims to treat everyone like a customer: patients, families and the medical professionals who refer patients.

Today, Summit Home Health & Hospice has over 50 employees, including physical therapists, registered nurses and certified nursing assistants who provide home health care to seniors and other home-bound patients along the Wasatch Front. Summit Home Health & Hospice became Medicare and Medicaid certified in December 2005.

The majority of Summit Home Health & Hospice patients need health care on a regular basis but have difficulty leaving their homes. We provide regular home visits and teach patients how to care for themselves, including how to take medication and administer IV therapy.

Because of our size, we are able to maneuver more quickly than a large corporation can. We work closely with doctors offices and other referral sources to complete paperwork and visit patients within just a couple of days of the referral.
